Home Health Care Services



Skilled Nursing
Wound care:
-  Teaching and monitoring
-  Wound Vac dressing changes
IV care:
-  PICC dressing changes
-  Weekly lab draws
-  Teaching and monitoring
Disease Education:
-  Teaching and monitoring new diagnoses
Colostomy:
-  Teaching and monitoring
Other non-skilled nursing:
-  Medication management
-  Safety monitoring and assessment
Home Care Aide/Personal Care:

Bathing/Personal care provided 2-3 times per week
based on need

Homemaker:

Light housekeeping
Meal Preparation
Local Errands
Local Grocery Shopping


Respite:
Caregiver breaks
Companionship  ​
****Services are provided in Louisa County only and availability is limited based on current staffing and patient needs.****

At this time we are unable to take any Medicare Advantage Insurance plans.

Health Promotion -
  • Childhood vaccinations through VFC Program of Iowa Health and Human Services
              (Participants must have NO Insurance or Insurance that does NOT Cover Childhood Vaccines)
  • Adult Immunizations
              (Influenza, Pneumonia & Shingles)
  • Tdap vaccinations for all ages
               (tetanus & whooping cough)
  • Medication Set-up
  • Oxygen level testing
  • Blood pressure checks
Special Programs
  • Newborn Family Home Visit Program
   (Transitional support for families with newborn infants in the home)
  • Partner's Program
          (Physician ordered testing and medication administration in our office for those who are NOT homebound)
  • Milestones Program
           (Provides funding assistance for eligible clients based on a sliding fee schedule)
  • Emergency Preparedness
           (Training and Resources)
  • Resource Information - Community Education
